Photography & Media Release

By checking the box below, I hereby grant Haloe Wellness ("Haloe") and its authorized representatives the irrevocable right and permission to photograph, film, record, and otherwise capture my image, likeness, and/or voice during events, activations, or interactions in connection with Haloe's activities.

I understand and agree that such photographs, video recordings, audio recordings, or other media may be used by Haloe for promotional, educational, social media, and marketing purposes, including but not limited to websites, social media platforms, print materials, digital advertising, and other communications.

I understand that I will not receive monetary compensation for such use. I release Haloe, its employees, agents, officers, and authorized representatives from any and all claims arising from the use of my image, likeness, or voice for the purposes described herein.

This release applies to all media formats now known or hereafter developed. I am 18 years of age or older, or have obtained parental/guardian consent if under 18.

Consent & Waiver

Wellness Participation Consent & Release of Liability

By checking the box below, I acknowledge that I am voluntarily engaging with Haloe Wellness ("Haloe") and its services, events, demonstrations, or wellness activations. I understand that Haloe's services are for general wellness purposes only and do not constitute medical diagnosis, treatment, or advice.

I represent that I am in good health and have no medical condition that would prevent my participation. I acknowledge that any wellness information shared by Haloe representatives is educational in nature and I should consult a qualified healthcare professional before making changes to my health routine.

In consideration of my participation, I, on behalf of myself and my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue Haloe, its owners, officers, directors, employees, agents, contractors, and volunteers from any and all claims, demands, damages, actions, causes of action, or suits of any kind, arising from or related to my participation in any Haloe event, service, or activity.

I have read this waiver carefully, understand its contents, and sign it voluntarily and of my own free will. I am 18 years of age or older, or the authorized parent/legal guardian of the participating minor.
